Keep all your medical information attached to your Accountability Tag(s) in case of an emergency situation.

DATA FORM INSTRUCTIONS:

1. Fill out the medical information form. USE PENCIL for ease of updating.

2. Fold the information form, placing data form so your Name and I.D. Number face outward, and so it fits into the carrier. Close the flap to secure the form.

3. Peel adhesive back and attach to the back of your Acccountability Tag.
